I was not issued a ticket for a traffic violation, which was no insurance, mainly because I should the officer my proof of insurance, but still received a FTA 4 months later for failure to show insurance. Why is this?

Obviuously there is a problem somewhere. You should hire an attorney to check it out and make sure that the situation is resolved.
