Legal Question in Business Law in New Jersey

To Whom It May Concern:

I have two questions regarding the formation of a new LLC:

1. Is the NJ "Certificate of Formation" Form considered the same as the NJ "Public Records Filing for New Business Entity" Form?

2. I am in the process of filing my Company's NJ-REG, and I am not sure what Business Code to use. Here's what my Company plans on doing: Sell, distribute, and market my own brand of nutraceutical, dietary supplement, vitamin/mineral, herbal, and natural products, for both domestic and international markets. My Company will not be manufacturing its own products; I will hire U.S.-based contract manufacturers to produce/manufacture my own branded products. Will my Company be categorized under a Wholesale Business Code, Manufacturing Business Code, or Retail Business Code? If so, what specific Business Code? Note: I believe the US FDA categorizes dietary supplements as Food, and not the "conventional" pharmaceutical drug products (prescription and Over-the-Counter).
