Check this site, although I think there is a small fee. Or do a Guggle search. You can buy software for about $20 that has these forms at Staples or Office Max.

You shoulod not atampt to do this without consulting an attorney that practices specifically in the area of Landlord & Tenant law. This is a highly complex and sophiscated document, and if you do it wron or is incomplete it can have severe legal ramifications. DO NOT use a form lease that you find somewhere.
