Legal Question in Family Law in New York

I am a single parent of a 14yr old boy.We live in N.Y.His father and I were never married. And we have had no contact with him since my son was 4. no child supportin over 10 yrs. how do I change my sons last name to my maiden name?

You will need his consent to do this, after paternity is established if he did not sign the Acknowledgement of Paternity.
