what if the Dr. refuses to fill out the paper work for fmla that my company needs. What is the law regarding the dr? Is it mandatory that they fill out the forms or what steps can I take ?

Your doctor is under no legal obligation to fill out your company forms. If you feel that you need an FMLA leave to care for yourself or an immediate family member after exhaustion of your available sick and vacation time then it is time for you to seek another doctor or perhaps in your current physician's opinion you do not need an extended leave from your job.
