You would need to use a Bargain and Sales Deed w/ Covenants form to facilitate this transfer. If you are the sole owner currently on title, then you would act as the Grantor granting interest of the property to yourself again and the other person as the Grantees. This would then have to be filed and recorded in your county's department of land records along with the necessary tax transfer documents.

If there is a mortgage on the property, then you must first check with your bank (mortgagee) to see if there are restrictions on adding a person to title.

I would recommend consulting further with an Attorney so this can all be done properly. Feel free to contact my office for a free consultation.
