Legal Question in Family Law in Ohio

My Ex wife & I want/need to change the custody agreement of our dissolution. My Ex's employment, financial, and housing situation has taken a turn for the worse. She has decided to look for employment in another state and wants to turn custody over to me so that I can get them enrolled in school. We have submitted to the Clerk of courts a request for modification of the custody. My question is what forms and/or documents and what wording should be used on the forms? What will the courts want to see for this to happen?

They will want to see proof of a subtantial change in circumstance and any calculated changes in child support.
