Legal Question in Workers Comp in Ohio

my work told me that i had to fill out a FMLA form to keep my job and my position. im on workman comp i had surgry and im off work for 16 wks if not longer. what im asken is this true cause if so then im out of a job. cause fmla is only 12 week long and im out longer then that. i didn't think thats right. can you tell me if it is or not or what i can do
