Re: refused

Thankfully, I rarely hear stories such as yours where an employer refuses to supply workers' compnesation information to one of their injured employees. There are many alternatives, you may download an 801 occuaptional injury claim form from the State of Oregon Workers Compensation Division website or you may complete an 827 first report of injury claim form at your physician's office to start the claim process. You may also contact the Workers Compensation Division to inquire as to your employer's workers' compensation insurance carrier and their contact information. If you need any further assistance, feel free to contact me directly at my Salem office or at [email protected].
