Adoption and Termination are really big deals and the formal requirements must be followed with great care or else the adoption is subject to future attack. Please hire an attorney to help you do this properly so that you don't live looking over your shoulder. Most attorneys who handle termination/adoption will do so for a fixed fee, assuming that there is no trial before the court (or a jury) involved.
