Need help on how to ''word'' a statement in a divorce decree

I do not have a lawyer and I filed the divorce myself. I need to know how to state in my divorce that my husband as

signed the ''Waiver of Citation''. What do I write under the ''APPEARANCES'' and ''RECORD'' column.

I don't recommend doing your own divorce. There are law firms that will provide forms for a fee, but if you insist on doing it yourself, I recommend going to your local law library for a copy of a forms book on Texas Family Law. Good Luck.
