Legal Question in Personal Injury in Utah

Our church is conducting a sports camp this summer. We have online registration. Is it possible to post the participation waiver online with a "I agree" checkbox, or do we need to collect a hard copy signature from every participant?

I suggest that because of the potential liability you get a signed release and waiver from each participant and their parent if under 18.
