Legal Question in Insurance Law in Virginia

Title insurance

I am refinancing a mortgage through the same bank i have now. Found out that i have a lein on my property from 9 years ago. Since then i had refananced the property twice and now for the third time. The 2 tiltle companies never picked up on this lein and i never got anything from the suposed creditor and im still in the same place. My credit score is also in high 700's. I owe only for the mortgage and have no other debts. The original lein is from a home equity line that my ex wife had gotten. Now the title insurance company is trying to say that when my new loan pays off the exisisting loan that there not responsible .But i dont have it yet and this might effect me getting the new loan for that much lower rate. I just makes me angry that alot of people drtoped the ball. This closing comany was recommended to me. I trust the bank and them to do there jobs. And now i dont know whats next. Also if i owed money why the heck didnt anyone tell me. Now that im single i cant afford anymore. Plus i have douhts about the amount that they say that i owe
