The Maricopa County Superior Court has a great "self-help" division. You can go to the law library (2nd Ave & Jefferson) or obtain the forms via internet. However, there a lot of legal issues you may want to consider. Especially if you expect this will lead to dissolution and whether or not this is a contested action.
