Legal Question in Family Law in Arizona

Ok, I have a son by one man and an marrying another who wants to adopt my son. The father is willing to sign over complete rights and be removed from the birth certificate which will allow my fiencee to adopt him. What forms do we need to fill out? Both my fiencee and myself and the biological father. Can I get them anywhere? I live in the state of Arizona, Yavapai County. Thanks.

Contact the Yavapai County Attorney. The office will help you do the parental termination and adoption without charging any fees, so long as the child's father is willing to sign the papers. I don't know if you have to pay any Court costs, but I imagine you do.
