I would like to know what form(s) to use when suing someone for causing medical, personal property, and punitive damages as a result of a car accident. Thanks so much for your timely response.

There is a form complaint you can use in the California Superior courts. If you do not wish to retain an attorney to represent you, you should at least have one assist you in filling out the complaint and related forms. Good luck on your case

You can use Personal Injury Complaint, neglience cause of action. There are no punitive damages allowed on personal injury negligence claims, unless the action was intentional. If you need assistance, please call our office.
