Hello, I am trying to fillout an affidavit form and having some trouble. Could you help me fill in the blanks please?

Before me, the undersigned authority, personally appeared _________________, who, being duly sworn, deposes and states upon his/her personal knowledge the following:

the foregoing instrument was acknowledged before me this ______ day of _____, by ____________ who is personally known to me or has produced identification and who did take oath

I believe that I need to have a witness for the bottom portion but, do I need a lawyer or can it be any witness?
