Child custody request

Can you tell me what i need to do to file a motion to change the custody of my son, from his mother to myself (father) & what form i would have to file? The judge advised that i do this in my case.

Filing a motion with the court is subject to sometimes complicated procedural requirements that must be complied with in order for the motion to be properly heard. You need to file a petition to change custody and you will need to prove to the court that it is in the child's best interests to do so. Not very easy. Be careful. If you can at all, you should get a lawyer.
