I am not sure you will find forms for this because custody modification is driven by the circumstances the child is in. Therefore, it would be hard to come up with a fill in the blank document. Also, there are different standards to apply to different circumstances.

If you are unable to afford an attorney to handle this for you, you can see if your county has a "modest means" program. Also, you can talk to the Circuit Court clerk and see if they can help you. Lastly, you might consider retaining an attorney merely to review your situation and give you specialized guidance.
