Legal Question in Immigration Law in Maryland

I am a Green Card holder. In 2008, I was written a ticket for Failure To Report An Auto Accident. The case went on Stet Docket in court in February 2009.

I am currently applying for citizenship, and in the application there are questions in Part 10.D regarding commiting a crime (Questions 15, 17, 19, 20). Would somebody please let me know what to answer in my situation?
