How to nullify / contest POW and Will?

A niece handling my 89 yr-old mother's medical matters and pays her bills, without my knowledge or consent, has become her POW (durable) and prepared her will and filed it.

My question is: how to reverse these actions and are her actions criminal?

The Power of Attorney will need to be revoked. This will need to be done in writing and notice is also needed.

The Last Will and Testament will need to be rescinded. This is accomplished by issuance of a new document and the appropriate filing.
