next of kin
Is there a free form that can be filled out to prove that you are next of kin to the banks, doctors, landlords or whoever, that can be used in Minnesota and Chicago or any state?

If there is property that needs to be administered, the best way to go would be to open an estate and get letters of administration. I'd recommend consulting with a local probate attorney about this.
