termination of parential rights

How would I go about terminating my ex-husband's parential rights (for a minor child)due to abandonment-ie; no child support for the life of the minor child, no effort at visitation or contact in over three years, whereabouts unknown to me,etc.. In spite of having a will stating what my desires are for the child's care incase of my death I am concerned that he might want to challenge the will because of some financial gain for himself.

As I am a single mother and have limited financial resourses I was wondering if there is a legal form that I could fill out myself in order to save on attorney's fees?

If you live in Clark County you can go to the self-hslp center at Family Court on Pecos and Bonanza. Otherwise try Nevada Legal Forms or findforms.com.
