Affidavit of Service

I have to file an affidivit of service. I have the form. I'm not sure if I have to let somebody else I know. This affidavit notarorized or I can do it myself, and send it along with the neccary paperwork. I'm not sure, if I need a process server,like when an affidavit of personal service is issued?

The key point is that if you are a person suing someone or the person being served (a party to the action) then you cannot serve the papers yourself.

The affidavit of service has to indicate: who is doing the serving; who is being served, where the service took place, why the server knew the person being served was that person.
