Short term disability

My son has a problem with his back and was referred to a pain management facility for treatment. This was not work related. The Dr. told him he could lift no more than 20 lbs. He worke d a few days after his ist treatment but could not do the lifting so was forced to stray home until things improved. When he took his forms into the Dr. to fill out the girls at the desk told him they couldn't fill them out because the Dr. wasn't liscened to do that. They told him he would have to have his family Dr. fill them out. I work in a Dr's office and I have never heard this. AAlso it has been our experience that insurance companies want the forms filled out by the treating Dr. Is this correct, you have to have a lisence to fill out insurance forms?

Your doctor should fill out the forms. he may be able to charge an amount for doing this which I feel he shouldn't. Why not talk to the doctor personally
