how do you change a name on a deed to a house?

You execute [sign] a new deed, usually a Quitclaim deed if the property is not being sold, or a Warranty deed if it is.

Example: Currently title to the property is held by John Smith. He wants to convey the property to himself and Mary Smith. He executes a new Quitclaim deed to the property [properly filled out with legal description] to John Smith and Mary Smith. Now there is a new name on the property.

If he was selling to Mary Smith, then John would execute a properly completed Warranty deed to Mary Smith.
