Hello
There cannot be a Template to such Notice, though you will get such templates on internet, but you should never rely on them. These are purely subjective issues. You need to take advise of a Lawyer to draft such notice. Remember, for any legal proceedings notice is the base, if you make any mistake in issuing notice (wrong averment, any unnecessary admission, failure to disclose any fact) can be fetal in future litigation and may affect your right.
So, better consult a lawyer to draft a notice and send such notice through a lawyer, which will be more effective.
