What forms are needed for an emergency hearing for modifying a child visitation matter.

The same as for a regular modification of custody, you are just going to add "ex parte" to the front of it. The only true "ex parte" for such a modification is an imminent danger to the child. Otherwise, it should be heard on noticed motion or an "order shortening time" so shortened notice.
