Penalty for not filing a claim.
Is there a penalty when the company did not file your workers compensation claim?
I saw a specialist for back and kneck problems and I was told that it's caused by work.
I gave the doctor's note to my employer, but did not file any claim. I have to see a therapist for my kneck and back, but I'm still waiting for them to give me an insurance claim. This was three weeks ago.
What should I do?

Employers are suppose to give you a claim form when you report a injury and send you to doctor for treatment. You indicated you went to see a specialist but it is unclear Who sent you to the specialist. I strongly advise you to contact a worker's compensation attorney.
