what form do i use to transfer title/deed in a rental property i own in my name to a LLC that i own for liability purposes

Either a Quitclaim or Warranty Deed and a DR219 tax form

A properly prepared deed will transfer the property; you need to make sure that you do not get your insurance cancelled by doing so- you could lose the whole benefit you are seeking to accomplish by to doing so.

You should see an attorney in your area before doing anything.

You can use a quitclaim, though there are many other considerations before making the transfer.
