Minor Name Change

I have sole legal and sole physical custody of my daughter. Her mother has visitation rights. I'd like to have her first and middle name changed. Would this be difficult to do pro se/ with an attorney? Do I need the consent of her mother?

Hello. You will need mom's consent, and you should have a lawyer. The court that handled the custody matter should be notified since it retains jurisdiction of issues pertaining to your daughter, until she reaches the age of 18. That court may decide to

appoint a lawyer to represent your daughter in the change of name proceedings. Will be happy to discuss this further and in more detail.
