Legal Question in Family Law in Massachusetts

I am trying to get the father of my child to sign the birth certificate so my daughter can receive health insurance (my company needs the certificate). He is refusing. He is giving me ultimatums i.e I wont sign the certificate until I have a paternity or agree to visitation. I am trying to tell him that that all goes through the courts but he wants us to figure that all out on our own. We are not married. what do I do? Aldo, how do I start everything off? Is there a form to get that he fills out for a paternity test or do I have to set that up with the courts? Thank you in advance to the answer. All help is appreciated.

You can file a Complaint to Establish Paternity, with the Probate and Family court, by your zip code that would be with the Worcester Probate and Family Court in Worcester MA
