Legal Question in Family Law in New Jersey

Hello,

I have joint custody of my daughter and I have her every weekend. Her mother and I were never married and my daughter's last name is hyphenated. It is her legal birth name. I have on going problem with her mother not using my daughter's full name (dropping mine) when registering my daughter with doctors, pre-school, etc.. She is not even noting me as an emergency contact or filing me on the Privacy Act forms with the doctors. Is this legal? If not, what can I do about it?

Your best option would be to file a Motion and get a Court Order to get her to do what she is supposed to do. You might be able to do this yourself, but most people choose to have an attorney assist them with this sort of application to be sure it is done correctly.
