Regaining maiden name

At the time of my divorce I did not choose to take back my maiden name and now I would like to. What is the cost and how do I go about doing it?

One way is to file a motion with the divorce court asking for the issuance of a what is called a "Nunc pro tunc" order reinstating your maiden name as an omitted part of the divorce action. In essence, asking the court to treat the failure to restore your name as an oversight.
