Legal Question in Insurance Law in Pennsylvania

I was involved in a 4 veh chain accident car #3 over a yr ago. My car was totaled and I was in pretty bad shape and taken by ambulance to the hospital. we all hit each other in the rear but my vehicle sustained the most damage being hit by a large suv behind me. I was treated for lumbar spain and sever lower back pain and went to therapy for almost a year. I currently continue the therapy at home and have oral medication to take on an as needed basis. I did obtain an attorney not right away but when papers were served to me by the driver in front of me. That part was settled. Due to the nature of my injury and since the pain comes and goes depending on circumstances, my attorney did file papers against the driver behind me for pain and suffering, etc. We all have insurance. Her ins company refuse to settle with me for anything other than a minute amount. My attorney suggested to go the next step since my therapy will be on as needed basis. I was surprised to get in the mail today a letter from the person behind me attorney handing her defense.. what is the need for this and why isn't her insurance company handling the claim. I received a packet of papers from her attorney asking me all kinds of medical questions and personal information about me that needs to be returned in 30 days. Can you give me some input on what this is about? I thought since she has insurance the need for an attorney is not necessary ? What are some of the reasons she obtained an attorney when I was the injured party and she rearended me? Why isn't her insurance company handling her defense?
