Do I have to sign?

Can an Employer require an employee

during out-processing to sign a form

that promises he will not discuss/or share any information he has obtained

during his employment with any individual?

Not unless you havea a pre-existing agreement or you are paid to do so, or if such conduct would violate the confidentiality of information which would legally be considered confidential or privileged.
