There are forms you can fill out, but it's more than that. I can't teach you how to practice law in an email. Have you tried calling an attorney and having him or her do the lawsuit?

If you have a case that has legitimate value, you shouldn't have any trouble finding an attorney willing to do it on a contingency. Feel free to contact me if so. If it has no value, you shouldn't be doing it. Value means actual compensable damages, against a defendant with actual ability to pay a judgment.

If you persist in doing it on your own, you can find the basic boiler plate forms at the court clerk's office or online, but you also need to learn how to use them, how to comply with all the court rules, procedures and time limits, and how to 'practice law' in drafting all the court pleadings and conducting the hearings you will have leading up to trial. That is what attorneys do for a living. The defendant will likely hire an attorney to fight you who knows how to do so. The courts are not there to teach anyone the law or procedure, they expect and demand you know and follow it, whether an attorney or not.
