Prospective employer requires employee to sign authorization to withold monies from final check for repair or replacement of lost or damaged items, phone bill etc

From your description, such a document is illegal. Employers cannot require, as a condition of employment, to waive rights to something that is otherwise unlawful.

It also sounds like the employer has been burned in the past, or just wants to draw down the final paycheck so there's nothing left. I'd be concerned about taking that job if they're already worried about the FINAL paycheck, when you haven't even started.
