Legal Question in Family Law in Florida

I want to order a name change kit.

I was born in Ilinois and was adopted when I was 14 by my step father ( I am 45 now).

I want to change my name back to my biological name (my birth father's name).

I now live in Florida. I have lived here for 5 years.

I would like to know if I should order a name change kit for Ilinois or for Florida.

There is no topic below that fits my question.

Your name change must be filed in Florida, so you need a Florida name change kit. I'm not sure, but there may be free forms online at www.flcourts.org under the family law forms section.

One thing to be aware of: if the kit or the online forms say you don't need to file a fingerprint card, ignore it. The law changes tomorrow to say that all name change lawsuits must include a fingerprint card. You can probably get a fingerprint set done at the local police or sheriff's department.
