How do I caption a lawsuit?

I want to sue someone for an unpaid bill. The bill is in my business' name. Is it proper for me to put my personal name in the caption of the lawsuit, as sole owner of the business? Or do I just use my business name? Or can I put my personal name d/b/a ''xxx''?

Use your name and d/b/a unless you are a corp or llc.
