you need to insert an ad in news papers and then approach the concerned authorities to note the change in their records.

surname is not changed but u can stop using it.

surname is identity of ur sect/ antecedents/forefathers

Furnish an affidavit to this effect and also give a public notice for change of surname.

you need to make an affidavit, get it attested by executive magistrate or notary public and then publish the name change ad in two news papers, one regional and one english. Apply to passport office "in case you already have your passport" along with that affidavit and published ads to change your surname in the passport.
