Forced FMLA

Can en employer force or place an employee on unpaid under the FMLA if the employee has not requested or does not want the leave? Everywhere I've read about FMLA it always states that the employee REQUESTS it. I've never found a reference to employer placing an employee on unpaid medical leave. I need some kind of answer soon as I will be fired in a week when the 12 weeks run out.
