What is it? How do i fill it out? What happens if i do not turn it in to clerk of court?

You do not fill out the 406, "Notice Of Right To Have Exmptions Designated". The form you fill out is the AOC-CV-415, "Motion To Claim Exempt Property". It is a very important form that must be filed at the Clerk's office within 20 days after you receive the Form AOC-CV-406. (You should have been given a copy of the Form 415 at the same time you were given the Form 406). If you do not file it, you may not be able to protect your property from being taken to pay off a judgment. The instructions are self explanatory, but you should seek legal advice before filling out the form if you have time to consult with an attorney before it is due.
