emergent modifcation to visitation!!!

I am the custodial parent and seeking a modification of custody. Father is under the influence of methadone thru a legit drug rehab program, and is abusing other this and drugs. How do I get him on supervised visitation until he is completely drug free?

Hire an attorney, file a petition, bring all this to the attention of the court and ask for supervised visitation. It sounds to me as though this is a real safety issue and I urge you to hire legal counsel to present your case for you. While there are general forms available, if you get into court and do not appropriately put on your proof, you may not be able to protect your child(ren) the way you desire.
