Perjury - How to report?

If the other party signs a clause that reads, ''I declare under penalty of perjury of the state that the statements in Part II of this plan are true and correct'' and they are, in fact (a matter of court record,) not, what can/should I do about it?

This is the proposed parenting plan, and it is what somebody has proposed and would work within if the court approved it. Whether it is true is probably only provable by experience.
