Upon receiving a notice letter from my company stating untrue statements about my unpleasant service rendered to customers, i took a chance to write up a survey form and asked my customers to give me their feedback regards to my overall performance without the permission from my management and signed off as ''Management of (my company)''. My point is to prove myself being wrongly accused as far as for future reference records. Will my company sue me for taking action this way?

We can't guess what your company may do. I'd suggest that given you already have one strike (accurate or not), many employers would terminate an employee on the spot (or as appropriate under a contract) when they found out about the form and signature.
