Legal Question in Employment Law in Illinois

I was employed with a company for 3 years.I quit @ Jan.10,2010. the company had a health and wellness program in 2009. Each employee could complete 4 things for the program.If any or all of sections was completed,then that employee received so much money.Depended on what was completed.The money is not sent out until the 2nd quater of the following year.Also during the year $25 is taken out each week for this program.I have been told because I quit and am no longer an employee that they do not and will not send me any money. Everything was filled and filed during 2009 while I was still employed.I worked all of 2009 and feel I still should receive this money.

For the amount of damages you have, it's not practical to hire an attorney. You can go to the clerk of your local circuit court and fill out a simple Small Claims Complaint and Summons. The clerk will help you with the filing. There is no magic language you need to use for the complaint: just explain in your own words. The clerk will either mail the summons by certified mail or have the sheriff serve the summons. You will be a court asap.
