Legal Question in Family Law in Minnesota

I am a c0-conservator for my mother and we had to sell her house. We submitted the petition to sell real estate and we were granted permission to sell. We sold, but a month after the sale, we received a call from closing company saying the title transfer was rejected because the legal description of the property was incorrect. How do we fix this?
