Name change for minor

My daughter had a child out of wedlock. Her boyfriend's name is on the bc, and now he has kicked them both out and is moving to conneticut to his ex-wife. Can she change the baby's last name to hers, without the fathers consent, if there was no paternity affidavit signed at the time of the birth? Where would she check to see if an affidavit had been signed?

Best bet, file for paternity and ask the court for the name change, while at the same time, geting child support.
